Developmental Disabilities Services Coordinator

The Clatsop Behavioral Healthcare Developmental Disabilities team has an opening for a full time (40 hours per week) devoted DD Services Coordinator.

The successful candidate will develop the Individual Support Plan (ISP) for adults and children in I/DD services and facilitate ISP meetings. Successful candidate will also:

*Complete annual Level of Care for K-Plan and Waiver services

*Complete annual assessment and Notification of Rights

*Complete Plan of Care in eXPRS to reflect and approve funding

*Complete Notice of Planned Action when applicable and assist individual, provider, or family to file hearing request(s) if desired

*Meet every individual, in person, at home a minimum of 1x/year

*Complete monitoring of services per OAR requirement

*Complete required Progress Notes and billing in eXPRS

Essential Duties and Responsibilities
Essential duties and responsibilities encompass the required tasks, duties, and responsibilities performed as part of the job and are the reason the job exists. In addition to tasks listed above, this position will:

*Advocate for client rights and services

*Collaborate with community providers to provide resources and services for clients and their families, including attendance of IEP meetings and collaboration with Employment providers

*Monitor providers per OAR; assist with licensing reviews for comprehensive service providers

*Complete agency and state data forms

*Attend all required meetings and trainings

Education and/or Experience
The Services Coordinator must have knowledge of the public service system for developmental disabilities services in Oregon and at least: (a) A bachelor's degree in behavioral science, social science, or a closely related field; (b) A bachelor’s degree in any field and one year of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; (c) An associate’s degree in a behavioral science, social science, or a closely related field and two years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; or (d) Three years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competency, or housing.

Specific Job Skills and Requirements
*Knowledge and understanding of various developmental disabilities and how they impact individuals

*Knowledge of DD services, programs, case management activities

*Skills to interact with clients, their significant others and subcontracted providers

*Ability to communicate positively and productively

*Ability to manage time and work demands appropriately and effectively

*Ability to work independently

*Ability to access various electronic programs to complete data reporting requirements accurately and efficiently

*Must possess excellent verbal and written communication skills

*Must be a team player and have the ability to work successfully with a variety of personalities on the
CBH team and in the community
Preferred:
Bilingual

Employer paid benefits for full time employees include

12 paid holidays and 22 days of PTO years 1-3, increases after year 3. PTO accumulation starts at hire and is available for use as accumulated.

Elective Training Benefit includes $500.00 and 5 days paid annually for approved trainings (Licensed Clinicians receive $1000.00 annual training benefit)
CBH also offers sabbatical of 22 days after 10 years of service.
